Health Check by TeamSnap
LMLA requires a Health Check be performed for each participant prior to engaging in a session's activities. The TeamSnap Health Check is available for each event on a Team's Schedule. It is open 8 hours prior to a particular event's start time and takes less than a minute to execute the quick self-assessment. This saves time at the start of each session by allowing players to simply check in, rather than doing the Health Check manually by the Site Facility Manager.
TeamSnap does not store the specific responses to each question, only the status of whether the participant was cleared or not. The status of a participants clearance is only available to a Team Manager and in some cases Site Facility Managers. The status of a participant is not available to other players or parents. The status results are only stored for 90 days.

Protocol & Requirements for Player Healthy Return to Play During COVID-19
If a player has failed a Health Check or is returning to play from an illness, they are not permitted to return to play until they have received clearance in one of the following ways:
OPTION 1 – Physicians note indicating that you are cleared to Return to Play and completion of the LMLA Player Healthy RTP form. The physicians note must be uploaded as part of the RTP form completion requirements.
OPTION 2 – A call to 811 and completion of the LMLA Player Healthy RTP form verifying that the parent/guardian has followed the direction received from the Health Professional spoke to with regards to the player's symptoms. Health Professional Name and Date spoken with required as part of the RTP form completion requirements.
OPTION 3 – If the players has been tested for COVID-19, the parent/guardian must complete the LMLA Player Health RTP. An upload of the negative test result (screenshot or some other file) is required as part of the RTP form completion requirements.
